The Lundeen Medical Group is the "doing business as" name for multiple medical offices specializing in physiatry, physical medicine and rehabilitation, pain management, independent medical exams. Patients with industrial and occupational injuries, motor vehicle accidents, and intractable pain are treated. Workers' Compensation claims and personal injury cases are accepted. The practice specializes in C92 exams supporting the injured worker.

James E. Lundeen, Sr., M. D. is the owner and principal medical practitioner of the Lundeen Medical Group. Dr. Lundeen holds a BS in Electrical, Computer, and Biomedical Engineering from University of Illinois, Urbana, Illinois; MS in Chemistry from Illinois State University, Normal, Illinois; and MD from the University of Health Sciences, The Chicago Medical School. Dr. Lundeen has had post-graduate training at the Cleveland Clinic Foundation and St. Vincent's Charity Hospital in Cleveland, Ohio. Dr. Lundeen is boarded by the American Board of Independent Medical Examiners (August 2000).

The Lundeen Medical Group makes extensive use of computing resources. Each of the clinical locations and Dr. Lundeen's home (offices in his home) is fully networked. Four locations are interconnected using 56 kilobit DDS lines and a MUXserver. Each location has at least one PostScript laser printer, and at least one VT420 terminals. As a result, the practices have significantly fewer employees than other practices of similar size and complexity.

Significant features of the computing system which supports the operations of the Lundeen Medical Group practices are:

• very extensive patient, attorney, and payer search and browse capabilities
• extensive use of E-mail for communications and management
• a project management database which is fully integrated into E-mail; the system is called Fast Track
• electronic patient appointments including printing of next appointment slips; patient appointment book activities are fully integrated into employee work scheduling including the checking of travel times between remote office locations
• prescription writing via computer - includes extensive reporting capabilities include (DUR) drug utilization review reports
• referral writing via computer - includes printing of referral slip
• full integration of physician's and therapist's SOAP (subjective, objective, assessment, and plan) notes into word processing reporting and billing
• recording and tracking of non-financial events
• electronic billing to MCOs (Ohio Bureau of Workers' Compensation)
• full and automatic integration of patient medical database information into word processing reporting
• hierarchical menu system with attribute controlled access; control may be changed while system is running
• business support systems including electronic time cards, payroll, tax report preparation, account receivable management tools, risk management, and extensive reporting
• support of attached PostScript printers on dialup workstations
• remote FAXing of all clinical reports
• highly automated C92 report creation.
